How it Works

1. Request an appointment through the button above (fill out basic information and pick a time slot that works for you.) Please choose “out of pocket” when asked “How do you plan to pay.”

2. Once approved, you will be sent some online paperwork to fill out prior to our first consultation.

3. Once your scheduled appointment time arrives, simply click on the link for the video consult that is sent via email and you are good to go!
